IF YOU ARE REGISTERING A PLAYER (WHO HAS BEEN OFFERED A PLACE IN THEIR AGE GROUP SQUAD) FOR THE FIRST TIME PLEASE COMPLETE OUR PAPER FORM WHICH IS AVAILABLE FROM YOUR SON/DAUGHTER'S AGE GROUP TEAM MANAGER OR HEAD COACH.

If you would like your child to join Old Pats but have not yet spoken to their age group team manager or head coach please e-mail admin@oldpats.co.uk with their first and last name, date of birth and your first and last name and mobile number and we’ll put you in touch with the right person.

Please check that your child's details are up-to-date especially head and shoulders photo, primary contact details (address, telephone, mobile, and e-mail), school, and doctor name and contact details. 

PLEASE HAVE YOUR PLAYER PIC READY TO UPLOAD AND ALL OTHER INFORMATION AT HAND AS THE FORM WILL TIME-OUT IF YOU TAKE TOO LONG AND DATA ENTERED SO FAR WILL BE LOST.

PLEASE NOTE THE DETAILS YOU ENTER IN THE PARENT/GUARDIAN 1 SECTION WILL BE THE PRIMARY CONTACT DETAILS THE CLUB WILL USE TO CORRESPOND WITH YOU.
